I am not 100% sure what the question is. However, I can vaguely grasp that
some customization of html elements is needed from the FormModel, which I
believe is best addressed here:
https://docs.djangoproject.com/en/2.1/ref/forms/widgets/ (cf. Customizing
widget instances).

If I have misunderstood you please try to make the question a bit more
clear please.
